REGISTRATION OF ESTABLISHMENT UNDER RULE 1020 OF THE OCCUPATIONAL SAFETY AND HEALTH STANDARDS

RATIONALE

  The establishment regardless of the size of economic activity, whether small, medium or large scale in one single location, shall register with the DOLE pursuant to the provisions of the Occupational Safety and Health Standards to form part of a databank of all covered establishments.

1. Duly accomplished form for Registration of Establishment under Rule 1020 (3 copies)
2. Department of Trade and Industry (DTI) Registration Certificate (1 photocopy) DTI

2. Submit to Action Officer the filled-out form with the complete documentary requirements.

2.1. Get the claim stub indicating the date of release of certificate.

2. Check the completeness of the form and documentary requirements.

2.1. If documents are complete and in-order, issue claim stub indicating the date of release of certificate.

2.2. If incomplete, return the Registration Form and documents to client indicating the lacking requirement/s and explain, as may be necessary. Registration is deemed not file

3.1. If not in order or if there is misrepresentation, endorsement letter to be sent to Legal Service for appropriate action.

3. Present the claim stub to the Action Officer on the scheduled date and claim Registration of Establishment under Rule 1020.
